Person-Centred Care

We use individual evidence based, outcome focused assessments to make sure our support is designed to enable people to live the life they choose.

Our trained staff uses the SPELL guide lines and Positive Behaviour Support frameworks to develop these support plans.  The plans are developed alongside the autistic adults who we support and, where appropriate, their family or an advocate, along with health and social care professionals.
Each plan is personalised and may include extra types of support, such as speech and language therapy, occupational therapy or psychological support. 

Each plan is built around 10 agreed outcomes:

A person’s overall support plan will be accompanied by other specific support strategies. This might incorporate a positive behaviour support plan. It is designed to help with individual challenges, for instance if someone is frequently distressed and at risk of harming themselves or others. This is developed in agreement with the autistic adult and, where appropriate, their family/advocate and health and social care professionals.

Each plan is personalised and may include extra types of support, such as speech and language therapy, occupational therapy or psychological support. We want each person to be themselves, be safe and fulfil their potential. This is the basis of everything we do.
